To apply for a Community Investment Grant, please follow the procedures below. An application to the Community Investment Grants Program consists of five main components:

- Demographic Profile (Excel) (submit as an excel spreadsheet, PDFs will NOT be accepted)
- Proposal Narrative (as a word document)
- Financial Information
- Board Roster
Applications must contain all five elements before they will be reviewed by EBCF staff.

II. Demographic Profile
To ensure that the East Bay Community Foundation is serving the needs and opportunities of our diverse communities, we collect demographic data on all applicant agencies and the population that they serve. Please complete the three sections on your board, staff and population. Please do not alter the spreadsheet.
III. Proposal (maximum 4 pages)
- A brief description of your organization and the project (if applicable)
a. Include overview and mission statement
b. Outline your experience in priority area and recent results. - Statement of constituents served and services offered
a. Include project, if applicable - Explain your role in the community,
a. Identify who supports your work, what business, city or county agencies support you as well as other community-based organizations (CBOs) you work with in making changes in your community.
b. Please identify current working relationships. - Outcomes
a. Identify at least 3 specific, measurable outcomes (include project outcomes, if applicable)
b. Describe how you will measure your effectiveness achieving your desired objectives.
c. Please describe how you receive feedback from those using your services.
IV. Financial Information
A. Agency Budget
a. Identify major revenue sources and expenses. Please include most recent audited financial statements in a pdf file (required for all organizations whose budget size is $500,000 and up).B. Project Budget
V. Board Roster
A. Include a roster of your agency's governing board; indicate officers and affiliations.

Application Review Process:
- Requests for grants will be reviewed by Foundation staff.
- Applicants for Community Investment grants will receive an automated e-mail when their application is received. Please add This e-mail address is being protected from spambots. - Staff will contact applicants to set up site visits or follow up as necessary.
- Applicants can expect a response within fourteen weeks of the grant deadline.
